34-Year-Old FemaleDiagnosis: Chronic neck pain, left shoulder pain, and headaches following concussion.Patient history: Patient involved in MVA 8 months prior. Patient had been treated in physical therapy for 12 weeks at two other providers, receiving therapeutic exercise and modalities, with only mild, temporary relief of symptoms. Significant findings: Patient presented with severely restricted cervical sidebending, rotation, and extension, with complaint of pain at end ROM. Left shoulder flexion and abduction were moderately limited, with left deltoid pain at end ROM. Left cervical musculature and left upper trapezius muscles were hypertonic and tender to palpation. Intervention: Patient was treated for a total of 6 visits with Strain/Counterstrain and therapeutic exercise for neuromuscular re-education. Outcome: After 2 visits, patient reported relief of headaches and 70% relief of shoulder and neck pain. Patient was discharged after 6 visits to an independent home exercise program, with no complaints. |
