39-Year-Old Female ElectricianDiagnosis: Right rotator cuff tendonitis.Patient history: Negative X-ray and MRI. Patient had been treated in physical therapy at another provider for 8 weeks, receiving therapeutic exercise and modalities, before returning to light duty work for 2 weeks, then full duty thereafter. Patient reported that full duty, overhead work, aggravated her condition. After repeatedly requesting further treatment, patient was referred to Advanced Rehabilitation Clinics. Significant findings: Patient presented with limited and painful right shoulder flexion. Right cervical rotation and left sidebending were limited and painful at end ROM. Right cervical musculature and right upper trapezius and levator scapula were hypertonic and tender to plapation. Intervention: Patient began treatment 2-3 times per week, receiving Strain/Counterstrain and AROM with no resistance only. Patient reported feeling 50% better after each of the first 6 visits, but pain returned with overhead work. The referring physician agreed to allow patient 3 weeks off work, returning to light duty for one week, then resume full duty. Patient was treated a total of 18 visits,receiving Strain/Counterstrain and therapeutic exercise to strengthen right upper extremity. Outcome: RETURNED TO FULL DUTY, NO RESTRICTIONS. 6 month follow up, no complaints. |
